This form can be completed electronically, reducing errors that often occur with handwritten forms. Please sign, email back to us at immunology@pathlab.co.nz
Once it has been reviewed and approved by Immunology (+/- pathologist), Pathlab will contact the patient to organise an appointment for the skin prick testing
Patient consent will now be provided by Pathlab and completed at the time of testing.
